fill it out completely, sign it and return it one of two ways:
1. FASTEST.
You may fax our SEND A PAYMENT form to our secure fax at 970-667-3538
2. OR, You may print out and mail it our SEND A PAYMENT form to us at:

3. To prevent identity theft we do not recommend that
you e-mail your credit card information unless the information is embedded in a file such as a jpeg.
After
we receive your payment we will email you to let you know we received your order and we will put your order into production.
